Integrated Community Services

The Integrated Community Services team provides short-term support after you are discharged from hospital to home.

Wheelchair and Posture Service

Wheelchair and Posture Service is a clinically-led service which meets the posture and mobility needs of people with a long term disability of all ages

Brain injuries, strokes & neurological conditions

The Rehabilitation Centre provides services for people suffering from acquired brain injuries, stroke, neurological conditions and chronic fatigue syndrome.

Community matron services

The teams deliver urgent and planned care to patients helping to prevent avoidable admissions to a main hospital and to promote a healthy lifestyle as well as to maximise independence at home

Interdisciplinary Teams who deliver nursing care at home (post-op care & rehabilitation)

Recovering in hospital

Our community hospitals provide in-patient services for patients who need hospital care but do not need to be in an acute hospital
