Please note that all referrals to community services need to be sent to SPR to ensure continuity of care. This is a secure pathway for our community services and ensures smooth transition into community services for our patients.
For any Virtual Ward or Rapid Response referrals please call us on 0333 358 4584 and go through Option 1.
Please note that VW and RR referrals cannot be accepted by email.
Types of patients regularly seen
The Single Point of Referral (SPR) team for Shropshire Community Health NHS Trust is an admin hub, providing a one point of contact service for referrals into the community services for Shropshire and Telford Patients. The team receives and processes referrals and distributes them on the appropriate service within the community.
We refer to a variety of community teams, these include:
- IDT Teams (Community Nursing)
For housebound patients with nursing needs. Including: Phlebotomy, continence pads assessments, catheter care, wound care etc. Nursing care at home - ICS Teams (Community Therapy)
For Community therapy and OT services for housebound patients/ patients not appropriate for outpatient’s clinics. Integrated Community Services - Social Services
Including home adaptations Family Connect / First point of contact | Shropshire Council - Respiratory Services
This service sees patients who have a confirmed diagnosis of COPD, have sustained worsening of breathlessness despite maximal therapy, and are aged 18 or over. Respiratory Care - Admiral Nurses
For carers of patients with Dementia. Dementia - support available for family carers - UCR (Rapid Response)
Family and self-referrals welcome - Virtual Ward Virtual Ward
Patients not seen by this service
We cannot offer nursing and therapy for patients who are able to travel to a clinic appointment for assessment or treatment. This service does not see children - as there is a specialist nursing team for children at home.
