School Nursing

This section aims to provide General Practitioners (GPs) and members of Clinical Commissioning Groups (CCGs) with some more detailed information about our services. The information below is updated regularly, but, if you need to find out more detailed information about a specific service or have a very specific question about the service or referral pathway, then please contact the service manager or the key clinician detailed opposite.

Referral Process

How does a GP refer in to this service?

A GP who wants to refer into this service is asked to complete the 'school nurse referral form' which is attached to this page on the right hand side, and send to appropriate school nurse base which is detailed on the form.

If you need any further advice or information, you should contact the appropriate team, directly, using the telephone numbers on this page.

School Nursing

Types of patients regularly seen

School nurses support the individual health needs of children and young people between the ages of 5 – 19 in mainstream Local Authority schools across Shropshire and Telford and Wrekin. We also provide support to parents and carers of school aged-children where appropriate.

School nurses are in a unique position to be able to provide a valuable link between health, education and home.
