Children's Continence Service
This page is for all care professionals who need the service referral information or other relevant documentation.
How do care professionals refer into this service?
Children and young people must have had a physical examination to rule out any red flags prior to being referred. Referrers will need to fill in the referral form that can be found in the Downloads and useful links section on this page.
You can check you are making an appropriate referral by downloading and completing the Continence Level 1 Checklist. If all the advice in Level 1 has been followed and there are still continence issues, please complete our referral form and send with the Continence Level 1 Checklist.
Please ask families to complete a bladder and bowel diary to send along with the referral. This will expediate the triage process.
The Referral Form, Checklist and Bladder and Bowel diary can be emailed to shropcom.continenceforpaediatrics@nhs.net or sent by post to Coral House, Longbow Close, Harlescott Lane, Shrewsbury, SY1 3GZ.
Referrers within Shropshire Community Health Trust can complete a Rio referral form and email the team to notify us of the completed form.
Anyone who needs any help with the forms or wants to speak to a member of the team should telephone the professional line 01743 7300031.
