Wheelchair and Posture Services

This section aims to provide General Practitioners (GPs) and members of Clinical Commissioning Groups (CCGs) with some more detailed information about our services. The information below is updated regularly, but, if you need to find out more detailed information about a specific service or have a very specific question about the service or referral pathway, then please contact the service manager or the key clinician detailed opposite.

Referral Process

How does a GP refer in to this service?

For submitting first or new referrals, it is important that GPs access a professional who has completed our accredited referrers’ course, or attend one of the half day courses themselves at the service. Contact the team using the number on this page for more information. The dedicated referral form (listed in the "Useful downloads" box) should be completed either electronically or posted back to the service.

GPs can refer someone who is already known to the service by telephone, email or by letter by contacting the team by using the numbers on this page.
