How do Care Professionals refer into this service?
Any healthcare professional can refer a patient to the Urgent Community Response Team when they believe the individual can be safely supported at home.
The aim is to prevent unnecessary hospital admissions by providing appropriate care closer to the patient’s home or within a community setting.
HCPs can make referrals through Shropshire Community Trust’s Single Point of Referral Team on 0333 358 4584
Types of patients regularly seen
The service aims to offer an urgent comprehensive clinical assessment for individuals experiencing an acute health crisis which would otherwise require hospital admission.
Typical conditions suitable for a two-hour urgent community response include, but are not limited to:
- Falls with no apparent serious injury, including those where lifting equipment is needed
- Decompensation of Frailty caused by a stressor event such as infections
- Urgent catheter care where there is a risk of admission
- Urgent end-of-life or palliative care support where core care services are unable to respond.
- Urgent support for diabetes management, such as monitoring post hypoglycaemic episode
- Unpaid carer breakdown which will result in healthcare crisis if left unresolved
- Reduced function and mobility requiring urgent equipment provision and reablement opportunities.
The service operated between 0800-0000 7 days a week.

Urgent Community Response provides time limited health and social care including
monitoring. This includes but is not exclusive to:
- Holistic Clinical Assessment
- Observations including blood and glucose monitoring
- Investigations such as ECGs and bladder scanning
- Phlebotomy & Cannulation
- Male and Female urethral catheterisation
- Nebuliser and Oxygen therapy
- Wound management
- Syringe Driver
- Administering pressure relieving equipment
- Arranging social care support to assist with personal care and ADLs
- Prescribing equipment to maintain independence
- Ongoing monitoring as clinically required, or onwards referrals to other appropriate services.
