The Community Dying Well Fund is supported by generous donations from loved ones, friends and members of the public. These contributions enable Shropshire Community Health NHS Trust staff and volunteers to enhance care and support for people with palliative and end-of-life care needs, and for those important to them.
The fund enables the purchase of additional support, equipment and comfort items that enhance compassionate, person-centred care and help people remain in their preferred place of care, whether at home or in a community hospital

Palliative and End of Life care is provided every day by all clinicians and carers. Patients are seen in their own home or in the Community Hospitals.
The Dying Well Funds have so far provided
Swan boxes - a Swan Box is provided to the loved ones of the dying patient in hospital and contains useful items to support them at this difficult time
Listening monitors to be used at end of life to support patients in their own home
End of Life information leaflets
Additional pieces of equipment otherwise not provided by the NHS
We could further enhance patient and loved ones ’experience at end of life through your kind future donations or fund raising.
