This page is about what happens to the information that Shropshire Community Health Trust collects about you whenever you come to see us or we when we talk to you by video. It also tells you how we make sure it is kept safe.

We collect information about you such as:

  • Your Name
  • Why you are coming to see us
  • Your birthday and year you were born
  • Your address
  • The name of the person who will generally bring you to your appointments
  • Your Family Doctor (General Practitioner/GP)
  • The reason that you are coming to see us
  • Any information that your family doctor, you or your family gives us
  • What we do to care for you

We do this so we can:

  • Keep you safe
  • Help you to stay healthy
  • Help you to be happy
  • Tell your parents/carers how your care is progressing

We keep this information very safe and follow special information protection rules and laws to make sure nobody can see or use your personal information unless we say it is ok.

Sometimes we have to share this information with other people we know who help us to look after you.

We tell your parents/carers what personal information we have about you, how we keep it safe, what we use it for and whom we share it with. Information is sometimes called data. Keeping personal information safe is called Data Protection.
