Types of patients regularly seen
The service sees patients who have a confirmed diagnosis of COPD, have sustained worsening of breathlessness despite maximal therapy, and are aged 18 or over. The service see patients who need and support with the following:
- Admission Avoidance
- Exacerbation Review
- Clinic Reviews
- Nebuliser Assessments
- Oxygen Assessments
- Pulmonary Rehabilitation
- Sleep Apnoea
- Supported Discharge
- Self-management education
Patients not seen by this service
The service currently does not see non-respiratory patients or those that are under 18 years old.
