Community equipment services (CES)

Community Equipment Services (CES) provides equipment that plays a vital role in helping people of all ages to develop their full potential and to maintain their health and independence. We provide a range of equipment from simple aids for daily living to more complex pieces of equipment within the home environment.

Our services

Community equipment service (CES) provide equipment for children and adults that can help facilitate rehabilitation, hospital discharge, admission avoidance and end of life care enabling patients to remain in the place of their choice.

If you feel you require a piece of equipment to assist you with your daily living, then contact your local healthcare professional e.g. a district nurse; occupational therapist or physiotherapist who will carry out an assessment to identify your needs.

CES have a clinical advisor who is available to give specialist advice on equipment to support the decision making of healthcare professionals.

Services we do not offer

CES do not provide ceiling track hoists or specialist social care equipment e.g. bathing equipment. Please see the links below for local councils and other useful organisations.

CES do not provide wheelchairs, therefore please contact the wheelchair and posture service on 01743 444051.

Frequently asked questions

I have equipment I want to return

If you have smaller pieces of equipment, you can return these by visiting the rear of the building at unit D6, Hortonwood 7, Telford, TF1 7GP.

If you have larger items e.g., beds, hoists, etc or smaller items you are unable to return, then please contact us on 01952 603838 to arrange collection.

If my equipment needs replacing, what do I do?

Please contact your health professional who will review your equipment and order a replacement if required.

CES staff are unable to provide equipment without an order from a health professional.

There is a fault with my equipment, what should I do?

Please contact CES on 01952 603838 where we will attempt to give verbal instructions.

If this does not fix the issue, we will request that a maintenance technician visits you at home.

How do I place an order for equipment?

Orders can only be accepted from health professionals.

If you are a health professional and are already registered on TCES, then please select your service user and add items to your basket for self-collection or delivery.

If you are a health professional and not registered, then please contact CES on 01952 603838 who will take some details and provide you with your log in.

How do I raise a collection notice?

Collection notices can only be raised on TCES via a health professional.

Please select the service user and opt for collection instead of delivery.

What is the Local Offer?

Service Description

The moving and handling service/family trainer aims to meet the moving and handling needs of people of all ages within Telford and Wrekin.

This is achieved through an advisory service, skilled clinical assessment, demonstration/advice on appropriate moving and handling techniques, where this results in moving and handling equipment provision demonstrations are carried out to ensure safe/appropriate use.

What do you provide to children and young people with an additional need and or disability 0-25yrs?

Do you provide support for parents and carers?

Yes we do provide support for parents and carers. We have a Family trainer whose role is dedicated to helping a family member who is the main carer

Who can use our service? Age? Disability?

Anybody who requires a moving and handling assessment.

How to access our service (inc. referral routes)

The service will accept any new referrals from any health professional who works for Shropshire Community NHS Trust or Social Care Staff Telford & Wrekin Council for Telford and Wrekin patients only. This is defined as a G.P within the Telford and Wrekin area. The moving and handling service is not commissioned in Shropshire.

Care agencies and self-referrals can be made if the patient/service user is known to the service, therefore having a referral made previously from the above sources.

Care homes/supported living need to refer through single point of referral or the social work team at Darby House.

When and where is our service available?

Monday to Thursday 9am to 5pm and Friday 9am to 4pm.

Moving and handling/family trainer assessments will be carried out as judged appropriate to the service users needs, in most cases the home environment needs to be assessed for space constrains, hazards, bed/chair clearance for some equipment and for equipment currently in place. On some occasions it is deemed appropriate for the service user to attend the demonstration room at Hortonwood in Telford, for a full assessment.
